Membership Requirements

  • Be between the ages of 6 to 18

  • Complete Application

  • Parent / Guardian Must Attend a New Parent Orientation

Membership CHECKLIST:

  • Application

  • Proof of Income

  • Copy of Insurance Card (Member)

  • If youth currently receive benefits or services under any of the programs listed below (Please Note: you will have to provide official verification to the afterschool/summer program):

  • Temporary Assistance for Needy Families (TANF)

  • Supplemental Nutrition Assistance Program (SNAP) (also known as Food Stamps)

  • Medicaid or Social Security Income (SSI)

  • Reduced or free lunch program at school – Note: This eligibility is only for single youth eligibility.

  • This is not applicable if the entire school population is awarded free lunch in universal eligibility.

  • Peachcare for Kids

  • After School Program

    • August to May from the hours of 2:00 PM to 7:00 PM

    • Annual fee of only $65
